School Nurse Information

The school nurse, or authorized personnel, must administer all medications, both over-the-counter and prescribed.  Please bring all medications to the school office.

We ask that all student injuries affecting your student's academic or activity ability be reported to the school nurse, as well as your student's coach or sponsor.

​PRESCRIBED MEDICATION:
Prescription medicines must be in the original container in which it was purchased, indicating the name of medication, dosage, time to be given, student's name and physician's name.

PRESCRIBED TREATMENT:
Treatments should be described on a written prescription form from the physician.

Student Illnesses

School Health Personnel will notify parents when a student needs to be sent home from school due to illness. Conditions requiring a student be sent home include: temperature greater than 100°F. ; vomiting; diarrhea; unexplained rashes; or on determination by the school nurse that the child's condition prevents meaningful participation in the educational program, or presents a health risk to the child or others, or that medical consultation is warranted unless the condition resolves. 

If the student contacts their parent by cell phone stating that they are sick, please send them to the health office for evaluation. 

Physical and Visual Examination

Evidence of BOTH a physical examination and a visual evaluation is required within six (6) months of entry into Kindergarten or in the case of transfer from out of state, to any other grade. 

A physical examination is also required prior to entrance into the seventh (7th) grade. The physical examination is to be completed by a physician, a physician's assistant, or an advanced practice registered nurse. A parent who objects to the physical examination may submit a written statement of refusal for his or her child. Waiver forms are available in the school health office. Additional physical examination requirements exist for students participating in athletic participation. 7th grade physicals are accepted as sports physicals for the 7th grade school year.
